Journey to Wellness: A Diabetes Prevention Program

This class provides the tools to support you in learning healthier habits while preventing diabetes and other life-long conditions; available at low-to-no cost.

Take Control Of Your Health

Journey to Wellness is the National Diabetes Prevention Program recognized by the Centers for Disease Control and Prevention (CDC). This program is for people who live with prediabetes, have been previously diagnosed with gestational diabetes, or score high on the risk test for diabetes. Our low-to-no-cost classes can lower your risk for developing type 2 diabetes by up to 58%.

Upcoming Class Schedule

Whether you prefer to learn in person or virtually, our program has options to support your needs. Interested participants are welcome to sign up, up to three weeks after a class series has started.

English Virtual Classes

  • Tuesdays starting January 20, 6 – 7 p.m.
  • Wednesdays starting January 21, 12 – 1 p.m.
  • Tuesdays starting February 24, 5:30 – 6:30 p.m.
  • Mondays starting April 6, 5:30 – 6:30 p.m.

Spanish Virtual Classes

  • Mondays starting January 26, 5 – 6 p.m.

We welcome provider referrals. If you believe your patient may benefit from one of our programs, please complete the provider referral form. Our team will contact your patient directly to discuss available options and help identify the best fit for their needs.
