Our Diabetes Self-Management and Support program is for people living with diabetes, empowering them with the skills and knowledge to make healthier life choices to manage their diabetes and other conditions. By taking steps now, you can control your blood sugar and prevent or delay diabetes symptoms.

What to Expect

This class, offered in English and Spanish, virtual or in-person, is available at little to no cost to you! Each class offers support, skills to keep your blood sugar in check, and self-empowerment to make healthier life choices. Topics include

  • Healthy eating strategies
  • Monitoring your blood sugar
  • How to read and use blood sugar numbers
  • Medicines and how they work in the body
  • Managing diabetes long-term
  • Enjoying activity at your level
