Properly managing diabetes is the key to staying healthy! Our Diabetes Self-Management and Support program is for people living with diabetes, empowering them with the skills and knowledge to make healthier life choices to manage their diabetes and other conditions. By taking steps now, you can control your blood sugar and prevent or delay diabetes symptoms.
Upcoming Class Schedule
Whether you prefer to learn in person or virtually, our program has options to support your needs. Interested participants are welcome to sign up, up to two weeks after a class series has started.
English In-Person Classes

We welcome provider referrals. If you believe your patient may benefit from one of our programs, please complete the provider referral form. Our team will contact your patient directly to discuss available options and help identify the best fit for their needs.
