How Can We Support You?
HCP connects children and youth with the care they need. Through working one on one with our HCP Care Coordinators, families gain a deeper understanding of their children’s condition and are equipped with skills and tools to support their children’s health care needs.
Coordinators can connect you with
- Developmental evaluations
- Primary and specialty care
- Community services
- Financial resources and insurance assistance
- Family support groups
- Transition services for youth moving into adulthood
- And answers to other questions affecting your family
Together, we identify and prioritize your child’s needs, support you in advocacy, and create a plan to help you reach your goals.
