Frequently Asked Questions

We can assist you with many applications, including:

  • CHP+ for children ages 18 and under, and pregnant women
  • Regular and emergency Medicaid programs for children and adults, from birth through 64 years old
  • Connect for Health Colorado marketplace enrollments during open enrollment or throughout the year due to a qualifying life event, such as losing a job, a change in income, marriage, divorce, or a household member turning 26.
  • Immediate and temporary access to coverage for children and pregnant women while their Medicaid or CHP+ applications are being processed
  • Adding a baby to Medicaid/CHP+ coverage if the mother had active coverage in one of the programs when the child was born

Proof of identity and lawful presence for each family member applying for coverage

  • Driver’s license or government-issued photo identification
  • U.S. passport
  • Social security number
  • Birth certificate <- Will want to hyperlink to birth certificate page
  • Certificate of naturalization (DHS forms N-550 or N-570)
  • Certificate of U.S. citizenship (DHS form N-560 or N-561)

Legal permanent residents must provide one of the following

  • Legal permanent resident card
  • Employment authorization card
  • I-94 if a refugee or asylee

Proof of income for all income-earning parties in a household

  • Bring at least one month of consecutive pay stubs from the current or previous month of work.
  • If no pay stubs are available, bring a typed letter from your employer on company letterhead showing gross income for the previous and current month, how long you have worked there, hours worked per week, pay per hour, and pay frequency. Make sure to include the name of the person signing the letter and contact information to verify the letter, if needed.
  • If you are self-employed, bring your business ledgers, mileage sheet(s), and documentation of your business expenses for the previous month.
  • If you have unearned income, you must know the monthly amount.

Medicaid offers up to 90 days of retroactive coverage from the application date. If you are requesting retroactive coverage, you must provide income for all dates you are requesting coverage.

Active coverage cards and documents

  • Current employer or private health insurance card
  • COBRA documentation
