Permits to Install, Repair, or Expand an OWTS

A septic permit is required for new installations, expansions, and repairs. Begin by applying for a septic permit and providing all required information below:

• Application for Permit to Install, Repair, or Expand an OWTS
• Site Evaluation and Design Document Checklist (recommended)
• Percolation Test Summary and Data Form (if applicable)
• Soil Profile Test Pit Log
• Conventional OWTS Design Worksheet (optional, if applicable)
• Tank Replacement Worksheet (if applicable)
• Lift Station Worksheet (if applicable)
• Site Plan Example

Permit applications may be emailed to [email protected]. Permit applications may also be mailed or submitted in-person during business hours to the Environmental Health office. The normal business hours for the Environmental Health office are Monday thru Friday, 8:00 AM to 12:00 PM and 1:00 PM to 5:00 PM, excluding holidays. The Environmental Health office is located at:
