Journey to Wellness (National Diabetes Prevention Program)
Journey to Wellness is a grant-funded year-long lifestyle change program recognized by the Centers for Disease Control and Prevention (CDC) that will give you the tools you need to become a healthier you.
This program is designed for people who live with prediabetes, have been previously diagnosed with gestational diabetes, or score a five or higher on the risk test for prediabetes. Classes encourage healthy lifestyle changes to reduce your risk of developing type 2 diabetes by 58% (71% for people 60 and older).

What to Expect
Great news! Our program comes at little to no cost to participants and is featured in English and Spanish. Our dedicated lifestyle coaches are committed to supporting your unique needs, questions, and concerns in every session. Your journey to better health starts with personalized care and support every step of the way! Topics include
- Healthy eating
- Strategies to eat healthier food options when eating out
- Being active
- Staying hydrated
- Healthy sleeping habits
- Stress management
- And so much more!
